R statistical consulting for public health research: a defensible workflow

How remote R statistical consulting should handle public-health questions, reproducible code, model checks, interpretation, privacy and handover.

8-minute guideUpdated October 5, 2026

R is valuable because the workflow can be scripted, reviewed and rerun. Credibility still depends on the study design, data provenance, assumptions and interpretation—not on using code instead of menus.

1. Begin with the estimand and design

State the population, exposure or intervention, outcome, comparison and time structure. Then define the quantity the analysis is meant to estimate. A cross-sectional association, prevalence estimate, risk ratio and time-to-event effect answer different questions.

Sampling, clustering, repeated observations, weights and eligibility rules must enter the plan before model selection. No R package can infer these decisions safely from column names.

2. Make the code reproduce the result

A project should separate raw, working and derived data; record package versions; set deterministic seeds when randomness is used; and generate tables or figures from the same approved analysis data. Manual edits after the code runs create an avoidable reconciliation risk.

The handover should include readable scripts or a report source with comments that explain substantive decisions, not only programming mechanics.

Review these:
  • Stable project structure
  • Data dictionary and inclusion rules
  • Versioned transformation code
  • Session or dependency record
  • Outputs generated from approved data

3. Match the model to the outcome and sampling

Continuous, binary, count and survival outcomes require different likelihoods and checks. Clustered or survey-weighted data may need design-aware variance estimates. Small samples, separation, sparse categories and missingness can make a familiar model unstable.

Pre-specify key predictors and plausible confounders from the research question and subject knowledge. Automated stepwise selection can amplify chance patterns and produce fragile estimates.

4. Report uncertainty and practical meaning

Present effect estimates with confidence intervals, units and a clear reference category. Translate an odds ratio or regression coefficient into the question it actually answers; do not silently convert association into causation.

Model performance, calibration and sensitivity analyses should be framed as evidence about the analysis—not as proof that a model will generalise to a new population.

5. Keep public-health data appropriately controlled

Remove direct identifiers where possible and review combinations that could re-identify participants. Agree transfer, named access, project isolation, retention and deletion before files move. Public reproducibility examples should use synthetic or properly governed data.

A remote consultant should be able to start with a codebook, a de-identified structural sample or a secure controlled environment when the full dataset cannot be transferred.
